Duplicate Client Records Report

Description

This report helps to identify duplicate client records, as identified by matching client last name and date of birth. This report can be exported as both a CSV and an Excel file. This is a daily report.

Use this report to identify potential duplicate client records.

Fields and Columns

Parameters

You can limit the report output by date.

Report Output

Each column of the EVV report displays:

Columns	Description
Account Number	the provider agency’s account number
Account Name	the provider agency account name
Payer	the payer ID
Client Medicaid ID	the state assigned ID or EOEA assigned ID
Client First Name	the client’s first name
Client Last Name	the client’s last name
Client DOB	the client's date of birth
