Here is how to request a device in eTrac.
- Log onto eTrac and select Request Devices.
- Select whether the device will be shipped to the Agency or to the Caregiver.
- Complete the correct shipping information.
- If shipping to the Agency, use the checkboxes to show the days the Agency is open and available to receive deliveries.
- Also, provide the latest time the Agency is open, and someone is available to receive and sign for the device. You will use the 24-hour clock when selecting the time from the dropdown menu.
- Next, include any special delivery instructions if needed.
- It is required that you watch the linked training video that gives instructions for how to complete this form successfully.
After selecting the link and viewing the video, use the checkbox to agree to maintain all devices according to ODM Specification.
- Once completed, scroll down to finish this ordering process by selecting: Request Device.
