Client Addresses

Description:

This report displays a list of clients by the selected filter, alongside account information pertaining to address and location.

Note: Stx ClientID (Santrax Client ID) is used in EVV (Electronic Visit Verification) for mutual and group home arrangements, where multiple clients share a phone number and/or address. The Stx ClientID separates individual clients in the system from others with shared account information.

Use:

This report can be used by the agency as an overview of key client account information.

Filters

This report can be filtered by:

Field	Description
Company	The selected company.
Location	The selected location.
Admit Type	The code for the client's admission.
Team	The selected team.
Client Status	The selected client status.
Coordinator	The selected coordinator.
Staff Manager	The selected staff manager.

Sorting

This report can be sorted by:

Field	Description
Client Name	Sorts the report by client name in ascending or descending order. Blank if unknown.

Report Output

Each column of the report displays:

Field	Description
Name	The client's name (last, first).
Chart ID	The client's Chart ID number.
Stx Client ID	The client's Santrax Client ID number.
Phone	The client's primary phone number.
SOC	The client's start of care date.
Address	The client's primary address.
County	The client address' county.
Total	The total number of addresses on the report.
