Individual Client Task Activity Report

Description:

This report displays a list of visits and tasks performed sorted by client.

Use:

This report can be used by staff or the agency to view which tasks are performed for specified clients.

Filters

This report can be filtered by:

Field	Description
Company	The selected company.
Location	The selected location.
Team	The selected team.
Coordinator	The selected coordinator.
Admit Types	The code for the client's admission.
Client	Client by name (last name, first name) or Sandata Client ID. When a partial name is entered the report displays results that begin with the entered characters.
Staff	Staff member by name (last name, first name). When a partial name is entered the report displays results that begin with the entered characters.
Payor	The selected payor.
Payor Plan	The selected payor plan.
Date From	The first date of a selected range of dates.
Date To	The last date of a selected range of dates.
Task Name	The selected task name.
Selected Visits	The type of visit selected.

Report Output

Each column of the report displays:

Field	Description
Company ID	The company, address and ID for the client.
Client	The client's ID number.
Client Name	The client's name (last, first).
Medicaid ID	The client's Medicaid ID. Blank if non-applicable or unknown.
Employee	The employee's ID number.
Employee name	The employee's name (last, first).
STX ID	The employee's Santrax ID number.
Discipline	The staff member's position.
Date	The date of the visit.
Proposed Start/End/HRS	The scheduled start, end and hours for the visit.
Actual Start/End/HRS	The actual start, end and hours for the visit.
Task	The code for and description of the task(s) performed.
Reading	Additional information or results for a task performed.
Total of Tasks	The total number of tasks performed for the client.
Total of Visits	The total number of visits for the client.
Total of Proposed Hrs	The total number of scheduled visit hours.
Total of Actual Hrs	The total number of actual visit hours.
